Basic Blue Rx
MII Life Insurance, Inc. is the underwriter for Basic Blue Rx, a prescription drug plan with a Medicare contract. Enrollment in Basic Blue Rx depends on contract renewal. MII Life Insurance, Inc. and each Blue Cross® and/or Blue Shield® plan are independent licensees of the Blue Cross® and Blue Shield® Association. You are eligible to enroll in Basic Blue Rx if you are entitled to Medicare benefits under Part A or enrolled in Medicare Part B and live in the plan service area. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party. You may be enrolled in only one Part D plan at a time. If you are enrolled in a Medicare Advantage (MA) coordinated care (HMO or PPO) plan or an MA private fee-for-service (PFFS) plan that includes Medicare prescription drugs, you may not enroll in a prescription drug plan unless you disenroll from the HMO, PPO or MA PFFS plan. You may enroll in a prescription drug plan if you are a member of a PFFS plan that does not provide Medicare prescription drug coverage, or a MA Medicare Savings Account (MSA) plan, or an 1876 Cost Plan.
Rights and responsibilities upon disenrollment
When you leave or end your membership in Basic Blue Rx it’s called a disenrollment. A voluntary disenrollment is when you decide to leave the plan. You can do this only at certain times of the year, such as if you want to change to another plan during the annual enrollment period. An involuntary disenrollment is when the plan must end your membership. Examples of this type of disenrollment are if you leave the plan’s service area for longer than 12 months or permanently move out of the service area, or do not pay the plan’s premiums.
If you are disenrolled for any reason, the plan will notify you of the date your membership will end and the reason for it ending. If you have questions about disenrollment, please call Basic Blue Rx at the number below.
