MII Life, Inc., a Medicare contractor, is an independent licensee of the Blue Cross® and Blue Shield® Association and the underwriter for Basic Blue® Rx, a prescription drug plan. Enrollment in Basic Blue Rx depends on contract renewal. Each Blue Cross and/or Blue Shield plan is an independent licensee of the Blue Cross and Blue Shield Association.

About using contracted network pharmacies.

Basic Blue Rx members must use contracted network pharmacies to access their prescription drug benefit, except under non-routine circumstances when they cannot reasonably use network pharmacies. Members who use pharmacies outside the network will pay any difference in the non-network pharmacy’s charge and the plan’s allowable charge. Types of network pharmacies include: retail, mail order, long-term care, and Indian/Tribal/Urban.

Members may enroll in Basic Blue Rx only during specific times of the year. For more information about enrollment periods, please contact Basic Blue Rx Customer Service.

Basic Blue Rx’s contract with CMS is renewed annually, so enrollment in Basic Blue Rx after 2018 cannot be guaranteed.

MII Life, Inc., a Medicare contractor, is an independent licensee of the Blue Cross® and Blue Shield® Association and the underwriter for Basic Blue® Rx, a prescription drug plan. Enrollment in Basic Blue Rx depends on contract renewal. Each Blue Cross and/or Blue Shield plan is an independent licensee of the Blue Cross and Blue Shield Association. You are eligible to enroll in Basic Blue Rx if you are entitled to Medicare benefits under Part A or enrolled in Medicare Part B and live in the plan service area. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party. You may be enrolled in only one Part D plan at a time. If you are enrolled in a Medicare Advantage (MA) coordinated care (HMO or PPO) plan or an MA private fee-for-service (PFFS) plan that includes Medicare prescription drugs, you may not enroll in a prescription drug plan unless you disenroll from the HMO, PPO or MA PFFS plan. You may enroll in a prescription drug plan if you are a member of a PFFS plan that does not provide Medicare prescription drug coverage, or a MA Medicare Savings Account (MSA) plan, or an 1876 Cost Plan.

When you leave or end your membership in Basic Blue Rx it’s called a disenrollment. A voluntary disenrollment is when you decide to leave the plan. You can do this only at certain times of the year, such as if you want to change to another plan during the annual enrollment period. An involuntary disenrollment is when the plan must end your membership. Examples of this type of disenrollment are if you leave the plan’s service area for longer than 12 months or permanently move out of the service area, or do not pay the plan’s premiums.
